Description


OE00DX43 Clinical Practicum and Proficiency Examination in Basic Level Prehospital Emergency Care (10 cr)
Prerequisites Successfully completed courses (or equivalent competence):
- Basic Level Prehospital Emergency Care (including the studies/competences mentioned in the pre-requisites)
- Clinical Decision Making and Patient Education in Non-urgent Emergency Situations 3 cr
- Clinical Practicum in Basic Level Prehospital Emergency Care I 6 cr
- At least three medical calculation exams in nursing studies
- Multisectoral Medical Studies 3 cr
- Simulation exercises in Basic paramedic situational leadership and responsibility 5 cr (including the studies/competences mentioned in the pre-requisites)
Objectives The student
1) acts in different roles in the basic level paramedic team on one and two unit missions in accordance with basic level obligations, value base and professional ethics.
2) is responsible for the emergency care of acutely ill and injured patients at the scene and during ambulance transport.
3) in non-urgent emergency care missions, is responsible for the assessment and management of patients' care needs at the scene of the incident or remotely.
4) acts as an expert in emergency care in liaison with the authorities and manages basic level emergency situations in liaison with other authorities.
5) masters the principles of safe emergency ambulance driving.
6) communicates and behaves professionally in learning situations, reflects on his/her learning and identifies his/her development challenges.

3) Ambulance driving training, 1 CR
Recommended optional programme components If necessary, the student advisor will recommend optional programme components for each student based on their individual study plan.
Accomplishment methods 1) Advanced clinical practicum in basic level emergency care ambulance
- 4 weeks practical placement in a basic level emergency care unit.
2) Proficiency examination in basic level prehospital emergency care
- Case analysis task in paediatric emergency care, theory test, practical competence
demonstration of practical skills and skills portfolio.
3) Ambulance driving training in the driving simulator
- Training on Skillster driving simulator according to student's individual schedule.
Execution methods 1) Advanced clinical practicum in basic level emergency care ambulance
- The student actively participates in the daily duties in emergency station.
- The student practices working in the role of paramedic 1.
- The student follows the training rules and the rules of the work community.
- The student requests and reflects feedback on his/her performance.
- The student demonstrates his/her competence by actively participating in training supervision and self-assessment, evaluation by the supervisor and through the internship assignment.
- The student familiarises him-/herself with the emergency care of children and adolescent through a written assignment.

2) Proficiency examination in basic level prehospital emergency care
- The student prepares for the proficiency examination by refreshing and studying basic level prehospital care course materials.
- The student demonstrates his/her competence by making of case-analysis of paediatric patients, a theory test, a practical demonstration of competence and a skills portfolio.

3) Ambulance driving practice
- The student conducts instructed ambulance driving training on a Skillster driving simulator at basic level prehospital emergency care studies.

- is responsible for the systematic examination, decision-making and treatment of the patient at the scene, the choice of the place of further care and the care during transport
- in non-urgent situations, acts as a health care expert in assessing the patient's condition.
- discusses the situation with the patient/client and/or relatives, taking into account the situational factors
- identifies basic life support disorders and life-threatening situations requiring emergency care
- is responsible for the delivery of care and the evaluation of its effectiveness and is prepared for complications.
- independently carries out medical treatment of the patient with basic medicines
- uses examination and treatment equipment safely and appropriately.
- guides the patient/client as appropriate in all situations
- records observations of the patient's condition, situation and events, in accordance with the regulations and criteria for recording care and effectiveness.
- reports to the emergency care physician, in accordance with the ISBAR model, the information relevant to the prescription of treatment and to the point of follow-up, the information relevant to the continuation of the patient's care.
- communicates with a partner and/or care team when caring for a patient
- applies CRM thinking and workplace safety when managing a situation
- assesses the adequacy of resources and makes additional alerts as necessary
- obtains additional information to build up a situational picture and maintain situational awareness and anticipates changes in the situation

- uses VIRVE talk groups and emergency communication tools when communicating with different authorities

Assessment Frameworks Not applicable
Further Information The exam included in the assessment of competences is assessed 0-5, the traineeship is assessed pass/fail.

Acceptance of previously acquired competences (AHOT) guidelines. Only for traineeships, the assessment of basic competences cannot be accepted:

A written AHOT assignment may be used to assess the internship if the student has
1) at least an upper secondary vocational diploma in health care (paramedic) or a diploma in emergency medical services, including paramedic studies, and
2) at least one year of work experience in emergency care. This experience must have been gained within the last three (3) years.

An Assessment of Prior Learning and Knowledge (AHOT) must be completed prior to the start of the Basic Paramedic Situational Leadership and Responsibility course and will be agreed with the course supervisor in the semester prior to the course. If previous paramedic studies are also taken, the competency tests will be combined according to the teacher's instructions. The assignments will be returned to Moodle. No request for an assignment will be made using the HOT form.

Accreditation of prior learning (AHOT) task Basic level paramedic advanced skills training:

Case description of a paramedic assignment in which you act as a paramedic 1. The task should address the following issues:
- Task response times, codes: alert, encounter, transport.
- Description of the initial situation when you arrived on scene.
- Examinations you performed on your patient, grouped according to the 5-step patient encounter model, working diagnosis and treatments.
- A literature review of how this patient should have been examined and treated (references according to the thesis guidelines).
- An analysis of how the task went in relation to the literature, and why the task went as the literature teaches or what factors contributed to the fact that you examined and treated the patient differently
- A reflection in which you justifiably evaluate your paramedic 1 role in this task, your own competence as a paramedic 1 and your working partnership.
The assignment is max. 15 pages maximum. You can use tables or graphs to summarise and illustrate.

Acceptance of prior learning and competences (AHOT): All basic level paramedic studies:
- The qualification and work experience requirements are the same as those in the Advanced Basic paramedic training guidelines above.
- the student will discuss the requirements for the AHOT with his/her tutor teacher or the paramedic teacher responsible for the assessment of competences
- the student will complete a written AHOT assignment as assigned by the paramedic teacher
- the student will take the basic paramedic theory examination and, if passed, the practical assessment of competence
