OE00DX52 Clinical Practicum and Proficiency Examination in Advanced Level Prehospital Emergency Care (15 cr)
Prerequisites Successfully completed (or equivalent competence):
- All courses in basic level prehospital emergency care studies and proficiency examination in basic level prehospital emergency care (includes pre-requisites for the courses)
- Advanced Level Prehospital Emergency Care and Emergency Medicine I (includes pre-requisites for the course)
- Intensive Care I

The student who already has a registered nurse degree or has advanced to final stage at his/her previous nursing studies discusses on his/her individual study plan and schedule with the tutor teacher or the teacher responsible of this course.
Objectives The student
1) is able to take responsibility for the emergency care of suddenly ill or injured patient at the scene of an incident and during ambulance transport.
2) In non-urgent emergency medical situations, is responsible for the assessment and management of patient care needs at the scene or remotely.
3) acts as an expert in emergency care in liaison with the authorities and leads emergency care situations in liaison with other authorities.
Content 1) Clinical practicum in advanced level prehospital emergency care, 12 cr
2) Proficiency examination in advanced level prehospital emergency care, 3 cr
Recommended optional programme components If necessary, the student advisor will recommend optional programme components for each student based on their individual study plan.

- 8 weeks of supervised training in advanced level ambulance
2) Proficiency examination in advanced level prehospital emergency care, 3 cr
- National theory exam of advanced level prehospital emergency care, demonstration of practical skills and skills portfolio.
Execution methods 1) Clinical practicum in advanced level prehospital emergency care
- The student acts supervised as role of paramedic 1 in advanced level paramedic unit.
- The student practices advanced level decision making, medication management, care interventions and situation management, liaison with authorities, recording and reporting.
- The student actively requests and reflects feedback on his/her own performance to
enhance his/her competences.
- The student assesses his/her competence through supervision in training, a learning diary, self-assessment and supervisor evaluation.
2) Proficiency examination in advanced level prehospital emergency care
- The student demonstrates competence by passing the national advanced level paramedic theory exam and a practical demonstration of competence.
- The student reflects on his/her professional growth during studies and needs for development.

Evaluation Criteria

1) The student
- acts in the roles of paramedic 1 and 2 in accordance with the duties and responsibilities of a paramedic and the values and professional ethics of paramedicine.
- can take responsibility for patient assessment, treatment decisions, care and documentation of care at the scene and during transport until responsibility for care is transferred to the point of care.
- makes treatment decisions and provides care based on reliable evidence and situational awareness in collaboration with a working pair.
- identifies and anticipates deteriorating patient and life-threatening situations requiring urgent care and prescribes a working diagnosis for the patient
- carries out treatment and medical management taking into account the patient's basic medication or treats the patient on the basis of the most severe symptom.
- complies with treatment guidelines and regulations and authorisations for out-of-hospital medical treatment
- systematically assesses response to treatment and anticipates changes in patient's condition
- uses emergency care and communication technologies safely during patient examination and treatment
- plans care during transport, taking into account the patient's condition, prognosis, circumstances and distance to the final point of care
- records observations of the patient's condition, situation and events, treatment decisions, treatment provided and effectiveness
- reports to the emergency physician, in accordance with the ISBAR model, the relevant information for prescribing treatment and to the point of onward care for seamless continuity of care

2) The student
- can take responsibility for the examination, treatment decisions, care and documentation of non-urgent emergency of out-of-hospital patients.
- systematically examines the patient at the scene and follows consultation guidelines before making decisions not to transport.
- assesses, by telephone and digitally, the patient's need for care and guidance and makes decisions in accordance with his/her obligations.
- provides care and service education to patients in a client-centered manner, taking into account safety and situational factors

3) The student
- can take responsibility for establishing a situational picture and acting as a situation manager in one or more emergency units
- can take responsibility for patient and occupational safety in emergency medical services
- acts as a on-scene situation manager in emergency care in a goal-oriented and patient-safe manner
- cooperates with the various authorities in both day-to-day and special situations (multi-patient situations, major accidents)
- applies CRM principles as a situation manager and supports the situation manager when acting as a partner or member of the emergency team
- as a situation manager, uses regulatory communication technology safely and securely

5) The student
- reflects on and justifiably assesses his/her learning, professional growth and development challenges
- justifies the roles and responsibilities of the paramedic/shift supervisor in coordinating and directing paramedic units and personnel
- justifies the importance of workplace interpersonal skills
- behaves in a professional manner in accordance with employee responsibilities and study guidelines
- gives and receives feedback in a constructive manner
- completes tasks within agreed timeframes
Assessment Frameworks Not applicable
Further Information The clinical practicum can be partially accredited if the student has five years of advanced level prehospital emergency care work experience in the last seven years (any maternity leave should be taken into account here). Proficiency examination in advanced level prehospital emergency care cannot be accredited.

ASSESSMENT OF COMPETENCE: Portfolio with a brief description of your career as a paramedic at advanced level (1 page) and a table of your objectives for clinical practicum (advanced level paramedic's competency requirements). In the table, the student analyses and evaluates his/her own competences for each objective.

The student asks for a deadline for portfolio and schedules a discussion time with the responsible teacher beforehand to go over the key points of the self-assessment and three case reports. No factual errors should be made in the case analyses discussed. The student must be able to demonstrate mastery of advanced level prehospital emergency care protocols.

If accepted, the clinical practicum will be 6 credits (four weeks). The clinical practicum includes an internship assignment and portfolio.
